In conjunction with CRMA endocrinologists, perform diabetes teaching to referred patients.


Establishes collaborative NP agreements with CRMA’s 5 endocrinologists. Works alongside these physicians on a daily basis and relies on them for advice and feedback

Creates and manages an outpatient diabetic teaching service to cover Natick, Framingham, and Marlboro. Travels to each location during the week to meet local patient demand.  Service shall be similar to ADA-certified diabetic education programs.  Helps establish clinical protocols and implements them.

Becomes an expert resource in specific facets of diabetes management, such as insulin injection, insulin pump management, self-monitoring, and diet/lifestyle modification.

Responds to needs of referring physicians for reports, feedback, and customization. Some physicians may want the NP to monitor treatment and some may prefer the NP stick to patient education.  Both needs must be accommodated.

Is available to schedule 7 hours of face to face time with patients M-F (8am-12pm, 1pm-4pm). Adapts to the office protocols for each location in which the NP works.

Follows CRMA and PCHI protocols for billing NP services.

Leads initiatives within CRMA to achieve managed care targets for diabetic outcomes measures.

Performs other related duties which may be inclusive but not listed in position description.


Nurse Practitioner.  At least 2 years experience in an outpatient primary care environment.  Experience managing complicated and non-compliant diabetic patients.  Prefer experience as a diabetes educator, though not necessary.
