How do I sign up with a CRMA primary care physician (PCP)?
First, call the PCP’s office to make sure he is still accepting new patients and accepts your insurance. If those two steps check out, you can book your appointment with the receptionist. Note that some insurance plans will require you to keep them updated on your primary care physician. If your plan has this requirement, be sure to notify your insurance company after you make your appointment to ensure proper coverage. Also, please call your previous PCP and request that they send your medical records to your new PCP.
What should I do as a new patient?
Please fill out the new patient paperwork ahead of time and arrive to your appointment 15 minutes early to take care of any administrative steps. Make sure to bring your insurance card, as we will need to photocopy it.
What insurance plans do you accept?
All CRMA providers accept the following insurance plans, however some may be closed to new patients for certain plans. This is often the case for Masshealth or plans related to Masshealth. Check with your insurance company or call the CRMA office to be sure.
