Population Health Manager
Overview
The Population Health Manager supports primary care physicians (PCPs) and practices in managing their panel of patients using various Population Health management tools, such as registries, protocols, reminders, and patient outreach. The Manager will develop and implement PHM programs to meet expectations of internal and external entities.
Key Areas of Responsibility
- Leads CRMA’s population health management programs.
- Lead CRMA’s efforts in Quality Assurance.
- Represent CRMA to external entities for PHM programs, in particular Partners Healthcare.
- Gathers and manages patient data using EMR population registries
- Generates reports that support PHM programs.
- Works as a key member of CRMA’s central administrative team
- Act as Project Manager for multiple projects at any given time.
- Manages CRMA’s training staff
- Manages CRMA’s Float Pool staff
Qualifications
- Bachelor’s degree or higher in Health Administration
- Proficient in data management and reporting. At minimum must be expert at using MSExcel. MSAccess knowledge would be a plus.
- Proven problem-solver with ability to multi-task
- Prior use of electronic health records and other health care information systems desirable. Knowledge of Epic would be a plus.
- Prior experience working in an outpatient physician office desired.
CRMA will consider exceptional recent college graduates.
